About the Role

As a Community Account Executive at HarmonyCares, you will play a vital role in enrolling eligible patients into our innovative healthcare program. Your primary responsibility will be to lead community outreach activities, building relationships with physician offices and healthcare facilities to engage shared patients into our program.

Key Responsibilities
  • Develop and execute community outreach account plans for assigned geographies, including research on community provider practices and organizational structures.
  • Conduct in-person visits to physician offices, hospitals, and skilled nursing facilities to educate community providers and engage patients into our program.
  • Collect, analyze, and report patient outcomes and experiences to community physicians.
  • Conduct telephonic outreach to eligible patients or caregivers to enroll patients in our program and support team engagement goals.
  • Be the face of HarmonyCares in the community, collaborating closely with physician leaders to support community relationships.
  • Leverage electronic tools, spreadsheets, and reports to facilitate research, identify innovative approaches, and document and present plans and progress.
  • Meet or exceed established monthly engagement targets.
Requirements
  • College degree or 5 years of progressive career growth.
  • 3+ years of related experience in sales, account management, community outreach, or patient engagement in a medical/healthcare industry.
  • An understanding of value-based care and its impact on patient outcomes.
  • Proficiency in Microsoft Excel, Word, and PowerPoint, with the ability to generate Excel reports.
  • A dynamic and engaging personality with excellent interpersonal, verbal, and written communication skills.
  • Detail-oriented, with the ability to think on your feet and respond with great follow-up.
  • Compassion, an interest in people, and a professional presentation.
  • Curious and a continuous learner, with a passion for solving problems and a get-it-done attitude.
Preferred Qualifications
  • Bilingual in Spanish, Mandarin, or other languages.
  • Comfort with CRM and EMR tools.
