Key Responsibilities:
- Assist clients in receiving services by telephone and in person, ensuring a high level of customer satisfaction.
- Develop and implement beneficial activities to support clients' well-being and goals.
- Conduct thorough assessments of clients and gather relevant information to inform care plans.
- Provide information and support to clients and their families, addressing their concerns and needs.
- Establish and maintain relationships with other agencies and services to ensure seamless referrals and support.
- Maintain accurate and up-to-date records and prepare reports to inform care planning and decision-making.
- Participate in training, supervision, and meetings to stay up-to-date on best practices and industry developments.
- Monitor clients for signs of child abuse and provide crisis intervention as needed.
Requirements:
- Master's degree in Clinical Social Work from an accredited graduate school of Social Work or Master's degree in counseling or other behavioral health discipline.
- Ability to work a flexible 40-hour workweek, scheduled to meet the needs of patients and families within the guidelines of the bargaining agreement.
- Basic computer skills, including MS Word.
- One to two years of inpatient social work experience preferred.
- COVID vaccination and booster required.
- Vermont State License.
