Job Title: Social Worker - Travel Assignment

Tact Staff is seeking a skilled Social Worker to join our team for a travel assignment in Brattleboro, Vermont.

Job Summary:

We are looking for a highly motivated and experienced Social Worker to provide clinical and case management services to patients and their families. The ideal candidate will have a Master's degree in Clinical Social Work and be registered on the Roster of Psychotherapists with the State of Vermont Office of Professional Regulation.

Responsibilities:
  • Provide clinical and case management services to patients and their families
  • Work collaboratively with the healthcare team to develop and implement treatment plans
  • Conduct assessments and evaluations to identify patient needs and develop individualized plans
  • Develop and maintain relationships with patients, families, and healthcare providers
  • Participate in multidisciplinary teams to coordinate patient care
Requirements:
  • Master's degree in Clinical Social Work from an accredited graduate school
  • Registration on the Roster of Psychotherapists with the State of Vermont Office of Professional Regulation
  • Experience working with children and adolescents
  • One to two years of inpatient social work experience preferred
