We are seeking a skilled and compassionate Clinical Social Worker to join our team in Brattleboro, VT. As a Clinical Social Worker - Inpatient Services Leader, you will play a crucial role in providing clinical and case management services to patients and their families. Job DescriptionThe ideal candidate will have a Master's degree in Clinical Social Work or a related field and be registered with the State of Vermont Office of Professional Regulation. You will be responsible for providing clinical services to patients across the lifespan, including children, adolescents, and adults. Your expertise will be invaluable in helping patients and their families navigate the healthcare system and achieve optimal outcomes.
We are looking for a candidate who is flexible, compassionate, and able to work effectively in a fast-paced environment. As a member of our team, you will be expected to provide high-quality patient care, collaborate with interdisciplinary teams, and contribute to the development of our clinical services.
Key responsibilities will include:
- Providing clinical and case management services to patients and their families
- Collaborating with interdisciplinary teams to develop and implement patient care plans
- Conducting assessments and developing treatment plans to address patient needs
- Providing education and support to patients and their families
- Participating in quality improvement initiatives to enhance patient care and outcomes
We offer a competitive salary range of $65,000 - $85,000 per year, based on experience. In addition to a comprehensive benefits package, we also offer opportunities for professional growth and development.
RequirementsTo be considered for this role, you must have:
- A Master's degree in Clinical Social Work or a related field
- Registration with the State of Vermont Office of Professional Regulation
- At least one year of experience in inpatient social work
- Excellent communication and interpersonal skills
- Ability to work effectively in a fast-paced environment
