About the Role
We are seeking an experienced Nurse Practitioner or Clinical Nurse Specialist to join our team as a RN Care Manager. In this role, you will work closely with complex renal patients in their homes, dialysis centers, and remotely, providing comprehensive care and support to improve patient outcomes.

This is a challenging yet rewarding role that requires strong clinical skills, excellent communication, and a patient-centered approach. As a RN Care Manager, you will:
- Conduct thorough assessments of patients' medical, behavioral, pharmaceutical, and social needs
- Create personalized care plans to address identified gaps in care and barriers to good health
- Coordinate care across multiple settings, including hospitals, skilled nursing facilities, and acute care facilities
- Provide education and self-management support to patients and their families
- Manage patients during transitions of care to minimize avoidable readmissions
About You
To succeed in this role, you will need:
- At least 1+ year of experience in case management or care management, preferably in a similar setting
- A current, unrestricted compact Registered Nurse license
- Excellent written and verbal communication skills, with a patient-centered approach
- Strong analytical and critical thinking skills, with the ability to prioritize tasks and manage time effectively

We offer a competitive salary range of $90,000-$110,000 per year, depending on experience, plus a comprehensive benefits package, including:
