About the Role:The RN Case Manager will play a critical role in delivering high-quality care to older adults in their homes, skilled nursing facilities, or assisted living settings. This position requires strong assessment, intervention, and teaching skills to maximize patient comfort and enhance the quality of life for patients and their families.
Responsibilities:- Provide ongoing assessments of physical, psychological, social, and spiritual needs of patients and families.
- Develop, implement, and evaluate plans of care, making necessary revisions to address all problems.
- Coordinate home care services, maintaining continuity of client care with other health professionals.
- Utilize assessment, intervention, and teaching skills to maximize patient comfort and enhance the quality of life for patients and their families.
- Strong desire and passion to serve older adults.
- Registered nurse with current licensure with the State Board of Nursing in state(s) in which he/she practices.
- A minimum of three years of nursing experience preferred.
- Critical care/oncology, long-term care, or medical/surgical background with strong knowledge of pain and symptom management preferred.
- Hospice/Home Care experience is a plus, but not required.
- Excellent interpersonal skills.
- Ability to work as a member of an Interdisciplinary Team.
- Strong assessment skills, experience with symptom management preferred.
- Strong oral and written communications skills.
- Computer skills helpful.
- Reliable transportation, current driver's license, and current auto insurance.
- Demonstrated compatibility with PHS's mission and operating philosophies.
- Demonstrated ability to read, write, speak, and understand the English language to communicate with all customers.
