Key Responsibilities:
  • Collect, organize, and analyze data to ensure accurate and timely completion of MDS assessments.
  • Collaborate with the interdisciplinary team to develop and implement individualized care plans that meet the unique needs of each resident.
  • Ensure compliance with federal, state, and local regulations, as well as company policies and procedures.
  • Provide education and training to staff on the MDS process and supporting software.
  • Participate in quality improvement initiatives to enhance the overall quality of care provided to our residents.
Requirements:
  • Current RN licensure in the State of Minnesota.
  • Previous experience in long-term care, preferably in a leadership or coordinator role.
  • Knowledge of Medicare PPS and ICD-10 coding.
  • Excellent communication and interpersonal skills.
  • Ability to work independently and as part of a team.
Preferred Qualifications:
  • ICD-10 coding experience.
  • Previous experience with MDS/CAA completion and care planning.
