Key Responsibilities:- Collect, organize, and analyze data to ensure accurate and timely completion of MDS assessments.
- Collaborate with the interdisciplinary team to develop and implement individualized care plans that meet the unique needs of each resident.
- Ensure compliance with federal, state, and local regulations, as well as company policies and procedures.
- Provide education and training to staff on the MDS process and supporting software.
- Participate in quality improvement initiatives to enhance the overall quality of care provided to our residents.
- Current RN licensure in the State of Minnesota.
- Previous experience in long-term care, preferably in a leadership or coordinator role.
- Knowledge of Medicare PPS and ICD-10 coding.
- Excellent communication and interpersonal skills.
- Ability to work independently and as part of a team.
- ICD-10 coding experience.
- Previous experience with MDS/CAA completion and care planning.
