Rehabilitation Nurse

Temple Rehabilitation Hospital is a state-of-the-art, 36-bed inpatient acute rehabilitation hospital dedicated to providing comprehensive care and specialized treatment to individuals recovering from severe injuries or illnesses.
Our rehabilitation programs are designed to maximize the health, function, and quality of life of our patients through ongoing care and customized treatment plans. We offer a range of services, including physical medicine and rehabilitation, to help patients recover from conditions such as stroke, brain injury, neurological disorders, trauma, spinal cord injury, amputation, and orthopedic injuries.
Job Summary:
As a Registered Nurse at Temple Rehabilitation Hospital, you will be responsible for providing direct and indirect patient care through the nursing process of Assessment, Planning, Intervention, and Evaluation. You will develop nursing care plans in coordination with patients, families, and interdisciplinary staff as necessary. You will communicate changes in patient condition with physicians, nursing supervisors/managers, and coworkers as appropriate. You will participate in discharge planning and ensure that patients receive comprehensive care throughout their rehabilitation journey.
Essential Functions:
- Conduct thorough patient assessments and identify patient needs.
- Develop and implement individualized nursing care plans.
- Provide patient and family education throughout the rehabilitation stay and prior to discharge.
- Provide care that is appropriate to the patient's condition and age.
- Collaborate with interdisciplinary teams to ensure comprehensive care.
- Participate in Continuous Quality and Performance Improvement (CQPI) initiatives.
- Maintain positive relationships with all customers.
- Promote rehabilitation nursing philosophy and scope of practice.
- Participate in team conferences and contribute to the development of individualized care plans.
- Document patient care and medical necessity in the medical record.
- Responsible for completion and revision of the Interdisciplinary Care Plan for each patient.
- Perform timely and accurate QI assessments.
- Provide a safe environment for patients, visitors, and staff and comply with approved safety standards.
- Support hospital leadership in a professional manner and act as a role model.
- Perform other duties as assigned.
- Graduation from an accredited Bachelors of Science in Nursing, Associate Degree in Nursing, or Nursing Diploma program.
- Current state licensure as a Registered Nurse.
- CPR/BCLS certification. ACLS preferred.
- CRRN preferred.
- Minimum six months' Medical/Surgical experience in an acute care setting preferred.
- Excellent oral and written communication and interpersonal skills.
