Key Responsibilities:
  • Provide case management services to youth in the program, including goal setting, progress tracking, and referrals to community resources.
  • Collaborate with an interdisciplinary team to ensure comprehensive support for youth and families.
  • Document all client interactions and track program milestones and outcomes.
  • Participate in agency-wide initiatives and collaborative projects to enhance services and outcomes.
Requirements:
  • Bachelor's degree in social work or a related field.
  • At least 2 years of experience working in human services.
  • Excellent communication and interpersonal skills.
  • Ability to work with diverse populations and engage youth and parents in service delivery.
  • Proficient computer skills, including Microsoft Office.
