Job Duties:

This role requires providing various levels of care to program participants, including assistance with daily activities, grooming, nutritional planning, and use of public transportation. The ideal candidate will have strong communication skills and the ability to work effectively in a team environment.

Essential Functions:

  1. Implements goals designed to maximize each program participant's level of independence, as determined by the IDT.
  2. Coordinates evacuation and ensures safety in actual emergencies and fire drills.
  3. Administers medication and/or monitors self-administration of medication as a certified AMAP.
  4. Responsible for crisis intervention and incident reporting procedures.
  5. Monitors and records progress on daily checklists, any behavior contracts, quarterly summaries, monthly goal forms, and progress notes.
  6. Communicates relevant information to other staff and supervisor(s).
  7. Meets routinely with people served and the management team to review goal progress, identify participant needs, and update program plans.
  8. Assists in researching and obtaining community resources appropriate and applicable to their needs and goals.
  9. Participates in moderate physical activities as part of recreation programs.

Requirements and Qualifications:

  • High School diploma or equivalency diploma required.
  • A valid New York State Driver's License with 2 years of driving experience is required.
