Role Overview

The Clinical Navigator plays a pivotal role in fostering referral connections, overseeing designated territories, and conducting patient evaluations. This position requires the ability to engage in sales outreach while analyzing referral and admission metrics from the assigned area. Collaboration with business development teams, nursing personnel, and various stakeholders is essential to enhance referral conversion rates. The Clinical Navigator also provides insights and suggestions for program enhancement and quality improvement initiatives, ensuring strong relationships with all involved parties, including patients, families, healthcare providers, and insurance representatives.

Key Responsibilities

Pre-Admission Evaluation

• Oversees patient referrals until they are admitted to the facility
• Coordinates and executes on-site pre-admission evaluations
• Reviews patient medical histories and gathers necessary admission details
• Maintains communication with the facility's Admissions department
• Conducts follow-ups with referral sources

Business Development Initiatives

• Develops and implements marketing strategies for the assigned area
• Applies sales techniques to demonstrate growth in admissions and revenue
• Identifies and nurtures potential referral sources
• Participates in regular Business Development meetings

Educational Outreach

• Identifies and informs new referral sources about available programs and services
• Organizes educational sessions for referral sources
• Coordinates tours of facilities for professionals, families, and prospective patients

Documentation and Reporting

• Maintains an accurate log of pending cases
• Provides regular updates on activities and market insights
• Curates a resource library of relevant articles and information

Qualifications

A minimum of two years' experience in a relevant field is required, ideally with a background in rehabilitation and marketing. Strong communication, clinical assessment, and leadership skills are essential.
