About the Role:

The Enhanced Support Specialist provides direct support, skill development, and community resource linkages to residents who have a psychiatric, substance use disability, and/or are within the legal system. This role is based on site meetings with residents to assess their needs and develop personalized service plans.

Key Responsibilities:

  • Provide person-centered support to residents, focusing on their individual needs and goals.
  • Assess and address the 8 dimensions of wellness, including housing, emotional health, physical health, social, employment, and educational goals.
  • Participate in the development and delivery of service plans, ensuring residents receive the necessary support to achieve their goals.
  • Advocate for residents and develop community-based support networks to promote independence and self-care.
  • Rotate on-call duties by phone, providing 24/7 support to residents.

Requirements:

  • High School Diploma or GED, plus 4 years of related work or life experience, or a Bachelor's degree in a mental health-related discipline.
  • Exceptional time management and organizational skills to support multiple residents.
  • Outstanding written and verbal communication skills to effectively collaborate with residents and colleagues.
  • Proficient in MS Office products and general computer literacy.
  • Ability to work collaboratively with individuals living with mental illness in a caring and professional manner.
  • Bilingual skills are a plus.
  • Valid NJ Driver's License with an acceptable driving record.
  • Lived Experience: Collaborative Support Programs prioritizes hiring individuals who have experienced mental health or substance use challenges, enabling them to inspire and support others.
