Rehabilitation Specialist

The Rehabilitation Specialist - Occupational Therapist role at JAG Physical Therapy involves assessing patient strengths and weaknesses, creating effective treatment plans, and aiding in their recovery process.
This position requires maintaining patient confidentiality, collaborating with an interdisciplinary team, and providing education and training to patients. The ideal candidate will have a Bachelor's or Master's Degree in Occupational Therapy, be licensed, and possess experience in Hand Therapy.
Key Responsibilities:
- Collects and records patient care data that follows format and guidelines of the patient care service, regulatory agencies and third party payers which reflects an understanding of reimbursement systems and their impact on patient care.
- Conducts initial evaluations, daily notes, reassessments/progress reports, discharge plans, and communication with members of the team.
- Prepares for evaluations in accordance with department policies/procedures and standards of practice including obtaining patient history, exhibiting knowledge of patient diagnoses, and researching unfamiliar diagnoses.
- Selects and administers tests and measures, synthesizing evaluation results to formulate the diagnosis, goals, prognosis, and plan of care.
- Establishes treatment plans based on evaluation results, goals, and best practice, collaborating with interdisciplinary teams, patients, and patient support systems.
- Identifies need for consultations and/or refers patients to another healthcare professional or health care services, initiates and completes discharge plans.
- Provides education/training to patients and/or patient support systems on functional training, neurological interventions, musculoskeletal interventions, and other relevant topics.
- Oversees support staff of Certified Occupational Therapy Assistants (COTAs) and Occupational Therapy Aides.
