Key Responsibilities- Provide direct patient care services to hospice patients and supportive care to their families
- Act as a liaison between physicians, patients, and the hospice team
- Develop and implement individualized care plans to meet the unique needs of each patient
- Collaborate with an interdisciplinary team to ensure comprehensive care
- Manage patient clinical records and participate in quality assessment and performance improvement programs
- RN license in the state of employment (or eligibility to obtain)
- 1+ years of nursing experience in hospice, home health, ICU, geriatric, or oncology
- Current drivers license and automobile insurance
- Current CPR certification
