Hospice and Palliative Care Specialist

We are seeking a skilled and compassionate Hospice/Palliative Medicine Nurse Practitioner to join our team in Chicago, IL. As a key member of our interdisciplinary team, you will provide high-quality, patient-centered care to individuals with serious, life-limiting illnesses, focusing on improving quality of life and providing support to patients and their families.
Key Responsibilities:
Conduct comprehensive assessments of patients to determine palliative care needs and goals.
Develop and implement individualized care plans in collaboration with interdisciplinary teams, including physicians, social workers, and spiritual care providers.
Manage symptoms related to serious illness, including pain management and emotional support.
Provide education and counseling to patients and families about illness progression, treatment options, and advance care planning.
Support patients and families through end-of-life decisions, ensuring respectful and compassionate care.
Participate in regular team meetings to discuss patient care and improve service delivery.
Maintain accurate and thorough documentation in accordance with regulatory and institutional policies.
Qualifications:
Active NP license in the state of Illinois
1 to 2 years of Hospice/Palliative Medicine experience (preferred)
Active DEA
