Palliative Care Nurse Specialist

The Palliative Care Registered Nurse is a vital member of the interdisciplinary team at VITAS Healthcare, responsible for providing expert, individualized coordination of care to patients and families.
Key responsibilities include:
- Completing comprehensive physical and psychosocial assessments of patients and families to identify needs and areas for intervention.
- Ensuring appropriate referrals to specialists and supporting adherence to treatment plans.
- Assisting patients in accessing community services and resources to improve their overall health and well-being.
- Educating patients and families to enhance understanding of disease processes and treatment options.
- Providing emotional support and counseling to patients and their families.
The ideal candidate will possess a minimum of 2 years of experience in an acute care setting, with a strong background in patient education, planning, and management. A current and valid license in the state of practice is required, with CHPN certification preferred. Reliable transportation and a current state driver's license are also necessary.
