About the Role

As a Case Manager at Baptist Memorial Rehabilitation Hospital, you will play a vital role in the interdisciplinary team caring for patients in the Memphis area who require continued care to recover completely. With a strong focus on patient-centered care, you will work closely with healthcare professionals to develop and implement individualized treatment plans that promote optimal health and function.

Responsibilities

• Completes departmental orientation and initial and annual competencies to ensure up-to-date knowledge and skills
• Collaborates with other departmental staff to identify barriers to care and develop solutions to ensure seamless patient care delivery
• Documents patient progress and completes the Individual Plan of Care (IPoC) per CMS guidelines
• Communicates with patients and caregivers to keep them informed of progress and provides information related to goal achievement, course of rehabilitation stay, and plans for discharge
• Coordinates weekly patient care team conferences to facilitate development, monitoring, and refinement of treatment plans to achieve identified patient goals and outcomes
• Reviews patient assigned CMG and identifies potential missed comorbid conditions that are actively being treated during the patient's stay

Requirements

• Current Registered Nurse or Social Work licensure or Healthcare professional licensure as Respiratory Therapist, Physical Therapist, Speech Language Pathologist, or Occupational Therapist
• Certification in Case Management or Rehabilitation Nursing preferred
• Minimum of 2 years social work or case management experience in an inpatient setting highly preferred
• Effective oral and written communication skills in English, additional languages preferred
• Basic computer skills in Excel, Word, Outlook, PowerPoint, etc. required
