Key Responsibilities:
- Complete departmental orientation and maintain annual competencies.
- Collaborate with other departments to enhance patient care delivery, identify obstacles to care or discharge, and develop effective solutions.
- Ensure timely and accurate documentation, including the Individual Plan of Care (IPoC) in accordance with CMS guidelines.
- Facilitate family conferences and maintain communication with caregivers to keep them informed about patient progress and rehabilitation plans.
- Coordinate weekly patient care team meetings to develop, monitor, and refine treatment plans aimed at achieving patient goals.
- Review assigned CMG for each patient and assist the team in identifying any overlooked comorbid conditions being treated during the patient's stay.
- Communicate findings to the Health Information Management team.
- Perform additional duties as assigned.
Qualifications:
- Current licensure as a Registered Nurse, Social Worker, or other healthcare professional (e.g., Respiratory Therapist, Physical Therapist, Speech Language Pathologist, Occupational Therapist).
- Preferred certifications in Case Management or Rehabilitation Nursing, such as CCM, ARN, ACM, or ANCC Board Certification.
- A minimum of two years of experience in social work or case management within an inpatient setting is highly preferred; experience in acute or rehabilitation hospitals is advantageous.
- Strong oral and written communication skills in English; proficiency in additional languages is a plus.
- Basic computer proficiency in applications such as Excel, Word, Outlook, and PowerPoint is required.
