Role Description

Concern Housing is in search of a Service Coordinator who will play a pivotal role in connecting clients with community resources, providing training in daily living skills, offering counseling and support, ensuring documentation accuracy, and maintaining compliance with program standards. The Service Coordinator serves as a vital resource and advocate for clients.

Key Responsibilities:
  • Assist residents in accessing community services such as case management, clinical support, vocational training, and entitlement programs.
  • Develop quarterly goal plans, conduct functional assessments, and facilitate goal plan reviews. Hold weekly meetings with clients to enhance skills and monitor progress. Maintain comprehensive documentation for client records, including progress notes and discharge plans.
  • Deliver individual counseling and support services tailored to client needs.
  • Provide housing-related assistance, including addressing maintenance concerns.
  • Conduct training in daily living skills, including medication management, household tasks, and financial literacy.
  • Perform monthly fire safety inspections to ensure compliance and safety.
  • Participate in weekly supervision and staff meetings to discuss client progress and program updates.
  • Stay updated with all agency-required compliance documents and training sessions.
  • Undertake additional responsibilities as assigned by the Program Director or Supervisor.
Qualifications:

A Bachelor's Degree in Psychology, Rehabilitation, Social Work, or a related mental health field, along with a minimum of two years of experience in a similar role or working with individuals with psychiatric disabilities. The ideal candidate will possess strong verbal and written communication skills and demonstrate sound judgment in client care.
