Role Overview

Concern Housing is looking for a dedicated Service Coordinator to facilitate connections with community resources, provide training in daily living skills, offer counseling and support, ensure proper documentation, and maintain compliance with program standards. The Service Coordinator serves as a vital resource and advocate for clients.

Key Responsibilities:

• Assist residents in accessing community services such as case management, clinical support, vocational opportunities, and entitlement programs.
• Develop quarterly goal plans, conduct functional assessments, and hold weekly meetings with clients to track progress and enhance skills. Ensure all necessary documentation for client records is completed.
• Deliver individual counseling and support services tailored to client needs.
• Provide housing assistance, including addressing maintenance concerns.
• Conduct training on daily living skills, including medication management, cleaning, and financial literacy.
• Perform monthly fire safety inspections to ensure resident safety.
• Participate in weekly supervision and staff meetings to foster team collaboration.
• Stay updated with all agency-required compliance documents and training.
• Carry out additional duties as assigned by the Program Director or Supervisor.

Compensation: $55,000 annually

Qualifications:

A Bachelor's Degree in Psychology, Rehabilitation, Social Work, or a related mental health field, along with two years of experience in a similar role or working with individuals with psychiatric disabilities. The ideal candidate will possess strong verbal and written communication skills and demonstrate sound judgment in client care.
