Job Summary:
We are seeking a skilled Licensed Practical Nurse (LPN) to join our team at a skilled nursing facility in Issaquah, WA. As an LPN, you will be responsible for providing high-quality patient care and working collaboratively with our interdisciplinary team to ensure the best possible outcomes for our residents.
Responsibilities:
- Provide direct patient care, including administering medications, performing wound care, and assisting with shower aide duties
- Conduct vital signs, monitor patient conditions, and report any changes or concerns to the nursing team
- Collaborate with the interdisciplinary team to develop and implement individualized care plans
- Communicate effectively with patients, families, and healthcare team members to ensure seamless care transitions
- Participate in quality improvement initiatives and contribute to a culture of safety and excellence
Requirements:
- 1 year of experience in skilled long-term care or transitional care
- Experience with IV therapy, central line blood draws, and wound care
- Current LPN licensure in the state of WA
- BCLS certification
- Ability to work every other weekend
