Responsibilities- Conduct regular wellness visits for all residents, monitoring their health and providing necessary care and support.
- Identify and address any changes in resident health status, collaborating with the Resident Care Director (RCD) to ensure seamless care delivery.
- Document all pertinent information in the resident wellness file, maintaining accurate and up-to-date records.
- Communicate effectively with residents, family members, and healthcare providers to ensure medical needs are met.
- Assist in maintaining wellness files according to Sunrise policies and federal and state regulations.
- Provide clinical support and assistance to community team members as needed.
- Graduate of an approved college or school of nursing.
- Current state license as a professional Licensed Practical Nurse (LPN).
- Minimum of one (1) year experience in home health, assisted living, hospital, or long-term care environment.
- Demonstrated knowledge of nursing practices, techniques, and methods applied to health and wellness resident care, and federal, state, and local regulations.
- Ability to handle multiple priorities, possess excellent communication and interpersonal skills, and demonstrate good judgment, problem-solving, and decision-making skills.
