LHC Group is seeking a skilled Physical Therapist to join our team in providing high-quality home health care services. As a Physical Therapist, you will be responsible for assessing and evaluating patient care needs, developing treatment plans, and providing interventions to improve patient outcomes.
Responsibilities:
- Evaluate patient functional status, body systems, and physical therapy needs.
- Consult with physicians to develop therapy plans of care.
- Provide services within the scope of practice, in accordance with plans of care, and in coordination with healthcare teams.
- Observe and report patient responses to treatment and changes in condition to nurse supervisors and physicians.
- Treat patients to relieve pain, develop and restore function, and maintain maximum functioning using acceptable standards of practice and evidence-based techniques.
- Instruct and inform healthcare teams, patients, and family/caregivers regarding plans of care, functional limitations, and progress toward goals.
Requirements:
- Current Physical Therapy licensure in the state of FL.
- Current CPR certification.
- Must have a current driver's license and vehicle insurance, and access to a dependable vehicle.
