Our at-home physical therapists form strong relationships with patients, tailoring treatment plans to meet their unique needs and goals. Key Responsibilities- Collect and record patient care data, including initial evaluations, daily notes, reassessments, and discharge plans.
- Prepare for evaluations by obtaining patient history, researching unfamiliar diagnoses, and selecting and administering tests and measures.
- Formulate diagnoses, goals, prognosis, and plans of care, and establish treatment plans based on evaluation results.
- Collaborate with interdisciplinary teams, patients, and patient support systems, and identify the need for consultations or referrals.
- Initiate and complete discharge plans, provide education and training to patients and support systems, and provide functional training, including ADLs, IADLs, and therapeutic activities.
- Provide interventions that are safe, effective, and in compliance with JAG Physical Therapy policies.
- Bachelor's or Master's degree in Physical Therapy, or a Doctorate degree preferred.
- Licensed Physical Therapist in the state practicing in.
- Completed state-regulated CEU requirements in the appropriate state (NY, NJ).
- Valid CPR license, and experience/knowledge with EMR software preferred.
- Excellent oral, written communication, and strong clinical decision-making skills.
