About the Role

We are seeking a skilled Nurse Practitioner to join our Optum HouseCalls team, providing annual clinical assessments to patients in the comfort of their own homes.

Key Responsibilities
  • Conduct comprehensive in-home assessments, including evaluations of past medical history, review of systems, medication reconciliation, vital signs, and physical exams.
  • Perform evidence-based practice screenings, including point-of-care testing as appropriate.
  • Identify diagnoses to inform care management and active medical management.
  • Communicate findings to inform members' primary care physicians of potential gaps in care.
  • Intervene in urgent and emergent situations as needed.
  • Educate members on disease processes, medication, and compliance.

Requirements
  • Active NP license or ability to obtain by start date, with unencumbered licensure.
  • Active ANCC or AANP national certification in Family, Geriatrics, Adult, Adult-Gerontology Primary Care, or Adult-Gerontology Acute Care certification, or ability to obtain by start date.
  • Ability to complete physical requirements of the job, including lifting a 30-pound bag and navigating various dwelling conditions.
  • Reliable transportation for home visit assessments.
