Transplant Coordinator RN

We are seeking a skilled Transplant Coordinator RN to join our team at Providence St Vincent Medical Center. As a key member of our cardiovascular team, you will play a critical role in coordinating and managing ventricular assist devices (VAD) and cardiac transplant patients.
Key Responsibilities- Coordinate and manage VAD and cardiac transplant patients in collaboration with the program medical director and referring physicians.
- Provide exceptional patient care and support to ensure optimal outcomes.
- Develop and implement effective care plans to meet the unique needs of each patient.
- Collaborate with interdisciplinary teams to ensure seamless care coordination.
- Communicate effectively with patients, families, and healthcare teams to ensure informed decision-making.
- Education to meet certification, license, or registration requirements.
- Oregon Registered Nurse License upon hire.
- VAD coordinator experience.
- 3 years of clinical experience with VAD patient care.
