We specialize in medical case management and vocational rehabilitation services for clients facing severe illness, catastrophic injury, and/or long-term disability. As a Field Medical Case Manager, you will be responsible for coordinating client care and ensuring the best possible outcomes for our clients. With a competitive salary of $80,000 per year, this is an excellent opportunity for BSN, RN, MOTR/L or DPT professionals looking to make a difference in the lives of others.

The ideal candidate will have 5+ years of experience in ICU, ER and/or home health settings, with a strong background in critical thinking, communication and interpersonal skills. Proficiency in MS Word, Outlook and Excel is also required. **Job Responsibilities:**
• Formulate a continuous client care plan for optimal outcomes
• Perform ongoing assessments and evaluations to ensure patient progress
• Collaborate with physicians, providers and treatment teams to achieve desired outcomes
• Address system problems and barriers to effective client care
• Communicate with third-party payers to maintain continuity of care
• Stay up-to-date on resources and facilities available to patients and family members

**Qualifications:**
• BSN, RN, MOTR/L or DPT required
• Current Michigan Registered Nurse, Occupational or Physical Therapy license
• 5+ years of experience preferred
• ICU, ER and/or home health experience a plus
