Job Summary

We are seeking a skilled LPN to join our hospice team, providing direct patient care and implementing current nursing practices. The ideal candidate will possess excellent communication skills, a strong work ethic, and a commitment to delivering exceptional patient care.

Responsibilities
  • Provide direct patient care as defined in the State Nurse Practice Act
  • Implement current nursing practices following a comprehensive assessment and the plan of care
  • Document patient services accurately and timely to reflect the plan of care
  • Assess and provide patient and family/caregiver education and information pertinent to diagnosis and self-plan of care
  • Participate in coordination of hospice services, reporting identified needs to the interdisciplinary group
  • Provide appropriate pain/symptom management, evaluating patient response to treatments/medications
Requirements
  • Graduate of an accredited practical nurse or vocational nursing program
  • Three years nursing experience, with community health/hospice or medical/surgical experience preferred
  • Currently licensed as an LPN/LVN in the practicing state
  • Maintain a current CPR certification
  • Must be a licensed driver with an automobile that is insured in accordance with state and/or organization requirements and is in good working order
