Job Summary

We're seeking a skilled LPN to join our team as a Hospice LPN Care Specialist. As a key member of our care team, you'll provide direct patient care, implement current nursing practice, and participate in the coordination of hospice services.

Responsibilities
  • Provide direct patient care as defined in the State Nurse Practice Act
  • Implement current nursing practice following a comprehensive assessment and the plan of care
  • Provide accurate and timely documentation of patient services to reflect the plan of care
  • Assess and provide patient and family/caregiver education and information pertinent to diagnosis and self-plan of care
  • Participate in the coordination of hospice services, appropriately reporting the identified needs to the interdisciplinary group
  • Provide appropriate pain/symptom management. Evaluates the patient's response to treatments/medications
Requirements
  • Graduate of an accredited practical nurse or a vocational nursing program
  • Three years of nursing experience. Community health, hospice or medical/surgical experience is preferred
  • Currently licensed as an LPN/LVN in the practicing state
  • Must possess current CPR Certification
  • Demonstrates good verbal and written communication, and organization skills
  • Valid driver's license with an automobile that is insured in accordance with state requirements
  • Understands the philosophy of hospice concept and the needs of the terminally ill
