Job Summary:
We are seeking an experienced Home Health Registered Nurse (RN) to join our team in Hampton, VA. The ideal candidate will have a strong background in home health care, excellent communication skills, and the ability to work independently. Responsibilities:
- Provide comprehensive nursing care to patients in their homes, including assessment, planning, implementation, and evaluation of patient care
- Develop and implement individualized care plans that meet the unique needs of each patient
- Conduct thorough assessments of patients' physical and emotional needs, identifying areas for improvement and developing strategies for addressing these needs
- Maintain accurate and detailed records of patient interactions, including progress notes, medication lists, and other relevant information
- Collaborate with healthcare teams to ensure seamless coordination of care and achieve optimal patient outcomes
- Prioritize tasks effectively, manage multiple patients simultaneously, and maintain a high level of productivity while ensuring quality care
- Communicate effectively with patients, families, caregivers, and other healthcare professionals to provide clear guidance and support
Requirements:
- Current RN licensure in Virginia (VA) or eligibility for licensure by endorsement
- At least 1 year of experience as a Home Health Registered Nurse (RN) within the past 3 years
- Experience with OASIS and HCHB (Home Care Home Base) is highly preferred
- Able to provide 48-hour turnarounds and maintain a caseload of at least 6/30
- Ability to work independently and prioritize tasks effectively
- Reliable transportation and a valid driver's license
Work Environment:
