About the Role:

We are looking for a dedicated Physical Therapist to provide high-quality care to our adult and geriatric clients in the comfort of their own homes. As a member of our multidisciplinary team, you will work closely with nurses and other healthcare professionals to develop and implement individualized care plans that promote independence and improve quality of life.

Responsibilities:
  • Conduct diagnostic tests and measurements to assess client needs and develop effective care plans.
  • Implement manual therapeutic exercises, gait training, balance, and other interventions to promote client mobility and independence.
  • Educate and instruct clients and their families on rehabilitative care and activities necessary for optimal health and safety.
  • Accurately document client care management and services provided using state-of-the-art technology.
Requirements:
  • A current NJ Physical Therapist license.
  • A minimum of one year of recent work experience as a Physical Therapist.
  • Graduation from a program approved by the American Physical Therapy Association or the Committee on Allied Health Education and Accreditation of the American Medical Association.
  • Solid computer skills and prior experience with electronic medical records (EMR) preferred.
