About the Role

Amergis is seeking a skilled Physical Therapist to join our team. As a Physical Therapist, you will be responsible for providing high-quality patient care and rehabilitation services in accordance with the plan of care developed for each individual patient and physician's orders when applicable.

Key Responsibilities
  • Perform initial comprehensive assessments and reassessments to evaluate patient rehabilitation needs and work collaboratively with the physician and other healthcare team members to modify the plan of care as necessary.
  • Develop and implement individualized physical therapy plans of care in collaboration with the physician and other healthcare team members.
  • Assess and evaluate patient therapeutic, rehabilitative, and functional status, as well as home environment, and provide recommendations for improvement.
  • Provide physical therapy treatment and establish goals for patient recovery and rehabilitation.
  • Train patients, families, and caregivers in the use of therapeutic appliances and equipment.
  • Initiate physical therapy programs and instruct other personnel and family members on certain phases of physical therapy.
  • Supervise Physical Therapy Assistants and ensure accurate and timely documentation in accordance with the plan of care and company policies.
Requirements
  • Current licensure as a Physical Therapist in the state of practice.
  • Graduate of an accredited school of Physical Therapy.
  • One year of prior professional Physical Therapy experience preferred.
  • Compliance with all relevant professional standards of practice.
  • Participation and completion of Amergis' Competency program when applicable.
  • Current CPR certification if applicable.
  • TB Questionnaire, PPD, or chest x-ray if applicable.
  • Current Health certificate (per contract or state regulation).
  • Must meet all federal, state, and local requirements.
  • Successful completion of new hire training as applicable to job site.
  • Understanding of patient confidentiality and HIPAA requirements.
  • Ability to effectively communicate with supervisors, co-workers, clients, and other stakeholders.
