Position Overview: The Bilingual Licensed Vocational Nurse (LVN) plays a vital role in delivering primary care nursing across all Petaluma Health Center locations. This position involves executing care plans, advising patients on health maintenance and prevention strategies, maintaining clinical records, mentoring fellow staff members, and providing case management in accordance with established protocols under the direct supervision of a Registered Nurse or Provider.

Key Responsibilities:

Patient Care & Education
  • Conducts nurse visits both in-person and via telehealth under the guidance of a Provider, which includes patient education on managing chronic conditions such as hypertension and diabetes, as well as basic follow-up care.
  • Performs comprehensive assessments and provides advice for patients of all ages under the supervision of licensed clinical staff.
  • Educates patients on the implications of abnormal lab results related to chronic diseases and public health concerns, utilizing approved educational materials.
  • Collaborates with providers to prioritize patient needs and enhance understanding of chronic disease management.
  • Processes medication refills according to established protocols.
  • Completes annual competencies to demonstrate proficiency in patient care and evaluates colleagues for skill validation.
  • Conducts medication reviews and adjustments in collaboration with the Provider.
  • Utilizes approved educational resources to inform patients about medications, wellness, and disease management.
  • Provides basic emergency care under the direction of a Registered Nurse or Provider.
  • Manages the inventory and disposal of controlled substances.
Documentation & Work Environment
  • Timely documentation of patient care in the electronic medical record.
  • Accurate documentation of triage assessments in the electronic medical record.
  • Reports any equipment malfunctions to ensure operational efficiency.
  • Maintains a clean and organized work environment.
  • Promotes ongoing improvements in workplace safety and environmental practices.
Case Management
  • Engages in indirect case management activities, including population health initiatives and follow-up care.
  • Provides direct case management for patients with uncontrolled chronic conditions under the direction of a Provider.
Training and Mentorship:
  • Assists in training sessions for staff.
  • Guides Medical Assistants in non-appointment tasks.
  • Mentors and orients new staff members, students, and team members.
Qualifications:
  • Equivalent to an LVN degree from an accredited vocational nursing program.
  • One to two years of progressively responsible nursing experience, preferably in community health settings.
Licenses and Certifications:
  • Valid California Licensed Vocational Nursing License.
  • Valid California driver's license or ability to obtain one.
  • Current Basic Life Support (BLS) certification or ability to obtain.
  • Must meet Petaluma Health Center's credentialing and privileging requirements.
Language Proficiency: Bilingual in English and Spanish, both written and verbal, is required. Proficiency will be assessed through a language proficiency test.
