Senior Healthcare Provider

As a vital member of the Family HealthCare Network, you will be responsible for delivering primary care medical services within a Patient Centered Health Home framework, emphasizing accessibility to healthcare.
Key Responsibilities:
- Deliver primary care medical services while adhering to established productivity benchmarks.
- Conduct thorough examinations and evaluate relevant laboratory, referral, and imaging results to establish accurate diagnoses.
- Document patient visits meticulously using designated templates and record pertinent data for diagnosis purposes.
- Meet budgeted visit targets consistently.
- Achieve clinical care objectives effectively.
- Ensure timely locking of medical records within the designated 72-hour timeframe.
- Fulfill Midlevel professional goals and objectives.
- Provide comprehensive and coordinated medical care for both acute and chronic conditions across all age groups, considering the family unit within a community context.
- Implement holistic healthcare practices focusing on health promotion, illness prevention, and addressing both protective and risk factors impacting patient health.
- Manage chronic illnesses and acute conditions, utilizing extensive procedural knowledge and coordinating care with specialists as needed.
- Advocate for healthcare access, emphasizing structured evaluations, early interventions, and health promotion initiatives.
- Collaborate with supervising physicians and team members for cases requiring additional insights.
- Promote and support the Patient Centered Health Home care model.
- Deliver exceptional customer service and patient care.
- Engage with patients and their families courteously and professionally.
- Maintain awareness of patient wait times throughout daily operations.
- Provide educational resources tailored to the health literacy levels of patients.
- Monitor patient outcomes based on relevant clinical quality measures.
- Actively participate in daily team huddles.
- Incorporate contributions from ancillary health team members in patient care management.
- Establish and oversee self-management plans for patients.
- Assist in ensuring compliance with clinical procedures and maintaining current clinical protocols.
- Complete chart reviews and peer evaluations promptly.
- Support the patient care team in performance improvement initiatives, staff training, and promoting Family HealthCare Network.
- Assist in evaluating team performance and quality enhancement efforts.
- Participate in the training of medical, physician assistant students, and dental residents as applicable.
- Engage in community events and health fairs organized by the Network.
- Contribute meaningfully in site and provider meetings.
- Meet all licensure and employment credentialing standards.
- Perform additional duties as assigned.
Professional & Technical Knowledge:
- Possess extensive formal training as a Nurse Practitioner or Physician Assistant, including a comprehensive understanding of the profession's theories and practices, typically acquired through a Master's Degree or equivalent advanced training program.
- Proficient in creating complex documents in Microsoft Word, including linking multiple files and embedding various objects.
- Skilled in utilizing advanced Microsoft Excel functions for database management and report generation.
- Adept at developing sophisticated presentations in Microsoft PowerPoint, incorporating embedded objects and transitions.
- Licensed as a Nurse Practitioner and/or Physician Assistant in the state of California.
- Ability to articulate professional opinions and synthesize information effectively.
- Capable of presenting information using visual aids such as tables, graphs, and charts.
- Experience in preparing and delivering presentations to large audiences.
While executing the responsibilities of this position, the employee is frequently required to sit and engage in repetitive hand movements for typing and grasping. The employee will also be required to stand or walk regularly and may occasionally lift or move items weighing up to 20 pounds.
Compensation includes a competitive salary range of $139,000 to $222,000, along with significant additional annual earning potential through outpatient productivity and quality incentives, generous retention bonuses, shift differentials, and extra shift pay. Additional benefits include sign-on bonuses, relocation assistance, annual CME days, and reimbursement, as well as comprehensive health and wellness benefits and retirement plans.
