Job Summary

We are seeking a highly skilled and compassionate Hospice Registered Nurse Case Manager to join our team at St. Croix Hospice. As a key member of our interdisciplinary team, you will play a vital role in providing high-quality, patient-centered care to individuals and their families.

Key Responsibilities
  • Patient Care
    • Conduct comprehensive assessments of patients and families to determine their unique needs and develop personalized care plans.
    • Provide professional nursing care, utilizing the nursing process to ensure optimal patient outcomes.
    • Collaborate with patients, families, and healthcare teams to coordinate care and address complex needs.
  • Communication
    • Effectively communicate with patients, families, and healthcare teams to ensure seamless care coordination.
    • Document patient progress and care provided, maintaining accurate and detailed records.
    • Participate in interdisciplinary team meetings to share knowledge and best practices.
  • Additional Duties
    • Participate in on-call duties as defined by the on-call policy.
    • Supervise ancillary personnel and delegate responsibilities as needed.
    • Stay up-to-date with industry developments and best practices through ongoing education and training.
Requirements
  • Graduate of an accredited school of nursing with 1-2 years of recent acute care experience in an institutional setting.
  • Current licensure in the State of Michigan.
  • Excellent observation, verbal and written communication skills, problem-solving skills, basic math skills, and nursing skills per competency checklist.
  • Prolonged or considerable walking or standing, ability to lift, position, and/or transfer patients, and ability to lift supplies and equipment.
