About the Role

Gentiva Hospice is seeking a compassionate and dedicated Registered Nurse to join our team as a Hospice RN/ RN Case Manager. As a member of our interdisciplinary team, you will provide high-quality patient care and support to patients and their families in their homes.

Responsibilities
  • Provide direct patient care services to hospice patients and supportive care to patients' families.
  • Act as primary liaison between physician, patient/family, and hospice team.
  • Manage and collaborate with an interdisciplinary group to provide comfort, peace, and dignity to patients and their families.
  • Evaluate patient and family needs and assist in the preparation and execution of the plan of care for each patient.
  • Maintain all assigned patient clinical records.
  • Participate in company quality assessment and performance improvement programs.
About You

We are looking for a Hospice Registered Nurse who is compassionate, kind, and empathetic to patients and their families. You should be comfortable providing care wherever the patient calls home and demonstrate flexibility and composure while supporting others in a time of crisis.

  • RN license in the state of requested employment (or eligibility to obtain).
  • 1+ years of nursing experience in hospice, home health, ICU, geriatric, or oncology strongly preferred.
  • Current drivers license and automobile insurance.
  • Current CPR certification.
