LPN Navigator Job Description

As an LPN Navigator with US Oncology Network-wide Career Opportunities, you will play a vital role in delivering exceptional patient care. You will be the right-hand support to our provider team, taking charge of a wide range of tasks to ensure the smooth operation of our practice.

Responsibilities:
  • Coordinate care during the full continuum of cancer care experience.
  • Observe, record, and report patient's condition and reaction to drugs and treatments to physicians.
  • Screen patients for appropriate information and instruct them in collection of specimen samples for testing.
  • Respond to and refer incoming telephone calls and instruct patients and family regarding medications and treatment instructions.
  • Phone in prescriptions and make other patient calls as required or directed by physicians.
  • Maintain patient files, records, and other information.
  • Participate in team meetings led by direct supervisor.
  • Provide education materials and support patient advocacy and empowerment.
  • Participate in community outreach and cancer prevention education.
Requirements:
  • High school graduate or equivalent.
  • Graduate from an accredited school for practical nursing.
  • At least five years of experience as a practical nurse.
  • Current LPN state license and registration with the State Board of Practical Nursing in VA.
  • Current BCLS Certification required within 3-months of hire.
