Overview:
Join our dynamic team as an LPN Navigator and play a vital role in delivering exceptional patient care As the right-hand support to our provider team, you will take charge of a wide range of tasks to ensure the smooth operation of our practice. From chart preparation and note review, to managing medication refills and coordinating patient care, your attention to detail and organizational skills will make all the difference. You will interact directly with patients, handle phone calls, oversee critical procedures like tumor testing and biopsies, and assist with new patient intake. Your dedication and expertise will be crucial in providing top-quality care and ensuring our practice runs seamlessly. Join us today and make a meaningful impact on patients' lives This full-time position offers a Monday to Friday schedule from 8:00 a.m. to 4:30 p.m. ET. Responsibilities:- Provides coordination of care during the full continuum of cancer care experience.
- Observes, records, and reports patients condition and reaction to drugs and treatments to physicians.
- Screens patients for appropriate information. Instructs patients in collection of specimen samples for testing.
- Responds to and refers incoming telephone calls. Instructs patient and family regarding medications and treatment instructions.
- Phones in prescriptions and makes other patient calls as required or directed by physicians.
- Maintains patient files, records, and other information.
- Participates in team meetings led by direct supervisor.
- Provides education materials.
- Supports patient advocacy and empowerment.
- Participates in community outreach and cancer prevention education.
- High school graduate or equivalent.
- Graduate from an accredited school for practical nursing.
- This position requires at least five years of experience as a practical nurse.
- Current LPN state license and registration with the State Board of Practical Nursing in VA.
- Current BCLS Certification required within 3-months of hire.
