Title: Triage Nurse

Supervisor(s): COO or designee

Department: Medical Services

Effective Date: June 18, 2024

Job Summary

The Triage Nurse supports patients and their families by providing clear, safe and effective telephone and in person triage using evidence-based processes and tools. The nurse on this team will blend critical thinking skills with a decision support tool enabling safe, standardized care to our patient population.

Duties and Responsibilities

  1. Respond promptly to each incoming call/visit and assist patients by providing standardized care and benefits navigation, while quickly developing a friendly, yet professional rapport.
  2. Conduct a thorough clinical assessment of symptoms and confidently determine the appropriate level of care required to safely meet the patient’s medical need and refer them using established guidelines.
  3. Follow standard procedures and protocols related to the triage service.
  4. Educate and communicate recommendations to patients thoroughly in patient-friendly language.
  5. Successfully route members to additional internal/external benefits and community resources, when needed.
  6. Serves as a central point of contact for all Included Health member emergency escalations.
  7. Participate in team meetings and continuous quality improvement.
  8. Prepare patients for medical examination, to include measuring and recording vital signs, chief complaint and other information into the Electronic Health Record.
  9. Assist the medical care provider during examinations and procedures.
  10. Draw blood and collect samples from patients and prepare specimens for laboratory analysis.
  11. Complete quality controls on lab equipment.
  12. Prepare, stock, clean and maintain equipment and rooms for patient visits.
  13. Monitor lab and other patient care supplies and make request for supplies purchase to Supervisor or designee when needs are present.
  14. Maintain OSHA standards in lab and all patient care areas, including cleaning logs.
  15. Maintain records of outside testing and ensure results are returned.
  16. Implement the orders of the medical care provider, including notifying patients of test results.
  17. Educate patients and/or their families about their condition, medication, and/or self-care skills.
  18. As needed, assist co-workers with answering the phone, registering patients, checking patients in and out, and billing.
  19. Appropriately handles patient complaints.
  20. Other duties as assigned.

Required

  • Maintain patient confidentiality at all times.
  • Be courteous and respectful to patients and co-workers at all times.
  • Ability to work effectively with providers, staff and third-party payors
  • Ability to work independently, establish priorities, and coordinate work activities
  • Ability to work under pressure

Credentials and Experience

Required

  • High School Diploma or GED
  • LPN
  • 2+ years’ experience in a triage setting, preferably some of that experience being focused on phone/in person triage, or 2+ years’ experience in an emergency room, or 4+ years’ experience in an ambulatory primary care role that included triage
  • Expertise in advanced clinical decision making
  • Comfortable working with a wide variety of medical conditions for both pediatric and adult populations

Preferred

  • Associate’s or Bachelor’s Degree in Nursing AND Active Unrestricted Kentucky License as a Registered Nurse

Special Requirements

Requires sitting, standing and walking for extensive periods of time. Requires working under stressful conditions or working irregular hours. Requires frequent exposure to communicable diseases, body fluids, toxic substances, medicinal preparations and other conditions common to a clinic environment. The employee frequently is required to reach with hands and arms. Specific vision abilities required by this job include close vision, distance vision, color vision, peripheral vision, depth perception, and the ability to adjust focus. Ability to work with a moderate noise level in the work environment is required.
