We are seeking a highly skilled and compassionate Primary Care Provider to join our team at Geriatric Specialty Care. As a Primary Care Provider, you will be responsible for providing high-quality, patient-centered care to seniors in our community.
Job SummaryThe Primary Care Provider will be responsible for the diagnostic evaluation, management, and therapeutic healthcare of primary care patients residing in assisted living communities, residential care homes, and private residences. This includes obtaining history and physicals, ordering and interpreting diagnostic tests, diagnosing and treating chronic conditions and episodic illnesses, and performing advance care planning.
Responsibilities- Conduct comprehensive health assessments, including past medical history, review of systems, medication reconciliation, and physical exams.
- Review and discuss need to close care gaps and quality measures.
- Perform and review standardized screening exams.
- Identify all existing medical conditions and accurately document assessment and conditions.
- Identify patients in need of chronic care management and pharmacy assistance.
- Document elements of personal prevention plans and health issues to follow up with PCP.
- Ensure a safe and smooth transition of patients following an acute or skilled stay or ED visit.
- Current Nevada PA or APRN license, credentialed with Medicare, Medicaid, and health plans GSC is actively contracted with.
- Minimum two years experience in specialty care area (Geriatrics) in private physician office, clinic setting, or community-based program preferred.
- Demonstrates clinical competence, including strong communications skills and medical knowledge.
- Remains current in licensure according to state regulation.
