About the Role

Pine Park Health is seeking a highly skilled Physician Assistant to join our team in providing high-quality, comprehensive in-home healthcare services to seniors in Northern Nevada.

Key Responsibilities
  • Conduct diagnostic evaluations, manage, and provide therapeutic healthcare to primary care patients residing in assisted living communities, residential care homes, and private residences.
  • Obtain patient histories, perform physical exams, order and interpret diagnostic tests, diagnose and treat chronic conditions and episodic illnesses.
  • Perform Advance Care Planning, accurate and timely documentation in the EHR, and coordinate with other members of the care team.
  • Conduct annual comprehensive health assessments in accordance with health plan partners and Medicare guidelines, including health assessments, past medical history reviews, medication reconciliations, and physical exams.
  • Identify patients in need of chronic care management, pharmacy assistance, and document elements of personal prevention plans and health issues to follow up with PCPs.
Requirements
  • Current Nevada PA or APRN license, credentialed with Medicare, Medicaid, and health plans PPH is actively contracted with.
  • Minimum two years of experience in specialty care area (Geriatrics) in private physician offices, clinics, or community-based programs.
  • Demonstrates clinical competence, strong communication skills, and medical knowledge.
  • Remains current in licensure according to state regulation.
  • Positively represents the practice by communicating with all visitors, patients, and staff in an enthusiastic, friendly, and professional manner.
