Position Overview: The Long-Term Services Support Coordinator will collaborate with a variety of diverse partner organizations across health and social service sectors. This role is dedicated to fostering consumer choice, self-management, and enhancing community inclusion, access, integration, mobility, and overall livability.

Position Responsibilities: You will be accountable for delivering comprehensive, person-centered support to participants aged 3 to 64 involved in the Commonwealth's Long-Term Services and Supports Community Partnership program. Your responsibilities will include facilitating the use of assistive technology, maintaining participant engagement, promoting problem-solving, managing data, and supporting outcome evaluations. Please note that home visits are a requirement of this position.

Qualifications:


• Bachelor's degree in Human Services or a related discipline, or


• An Associate's degree with a minimum of one year of professional experience in the field; Community Health Worker (CHW) or similar certification preferred.


• Bilingual candidates will receive priority consideration.


• Previous experience working with individuals with disabilities (children and adults) is advantageous.


• Attention to detail is essential.


• Ability to handle confidential and personal information appropriately.


• Proficient in computer skills.


• Strong writing and communication abilities.


• Reliable transportation and a valid driver's license are necessary for visits throughout the service area.

Hours: 40 hours per week, Monday through Friday.
