Community Services Coordinator

Job Summary:The Community Case Manager will provide support and assistance to families of children and adults who are eligible for DDS services, including those on the autism spectrum.
Key Responsibilities:- Provide support and assistance to families in obtaining services to meet their priority needs.
- Create opportunities for individuals to develop their skills, enjoy creative social experiences, and foster independence.
- Provide services to families and individuals according to DDS, CARF, and all applicable regulatory standards, and Bridgewell policies and procedures.
- Transport individuals as needed to and from medical, recreational, and other community activities.
- Maintain required documentation according to DDS and all applicable regulatory standards.
- Assist families with accessing resources and recreational opportunities in the community.
- Facilitate management of flexible spending funds as needed.
- Data collection and reporting according to DDS and/or Bridgewell requirements.
- Maintain core training certifications as mandated by DDS and Bridgewell.
- Bachelor's Degree in a related field or three years' experience working in the Human Services field.
- Minimum of 1 year of experience working in the human services field and/or experience working with families of children or adults with intellectual and developmental disabilities, including families of adults on the autism spectrum.
- Excellent written and oral communication skills.
- Excellent organizational and problem-solving skills.
- Valid driver's license and one year of driving experience.
- Basic computer skills.
- Bilingual/bicultural preferred, especially in Spanish or Khmer.
