Community Services Coordinator

Bridgewell is a leading provider of social and human services that empower individuals with disabilities and other life challenges to live safe, self-directed, and productive lives. We are seeking a highly motivated and compassionate Community Case Manager to join our team.
Job Summary:The Community Case Manager will provide support and assistance to families of children and adults who are eligible for DDS services, including those on the autism spectrum. The successful candidate will have excellent communication and organizational skills, with the ability to work independently and as part of a team.
Key Responsibilities:- Provide support and assistance to families in accessing services and resources to meet their priority needs.
- Create opportunities for individuals to develop their skills, enjoy creative social experiences, and foster independence.
- Provide services to families and individuals according to DDS, CARF, and Bridgewell policies and procedures.
- Transport individuals as needed to and from medical, recreational, and other community activities.
- Maintain required documentation according to DDS and all applicable regulatory standards.
- Assist families with accessing resources and recreational opportunities in the community.
- Facilitate management of flexible spending funds as needed.
- Data collection and reporting according to DDS and/or Bridgewell requirements.
- Maintain core training certifications as mandated by DDS and Bridgewell.
- Bachelor's Degree in a related field or three years' experience working in the Human Services field.
- Minimum of 1 year of experience working in the human services field and/or experience working with families of children or adults with intellectual and developmental disabilities.
- Bilingual/bicultural skills, especially in Spanish or Khmer.
- Valid driver's license and one year of driving experience.
