We are seeking a highly skilled and compassionate RN Case Manager to join our team at AccentCare, Inc. as a Home Health Registered Nurse. As a RN Case Manager, you will play a vital role in delivering high-quality patient care and improving health outcomes in the comfort of patients' homes.
Key Responsibilities- Conduct thorough assessments of patients' medical needs and develop personalized care plans to meet their unique requirements.
- Collaborate with healthcare professionals, family members, and caregivers to ensure seamless care coordination and effective communication.
- Provide education and support to patients and their families on disease management, medication adherence, and healthy lifestyle habits.
- Monitor patients' progress, adjust care plans as needed, and ensure timely interventions to prevent hospitalizations and readmissions.
- Document all patient interactions, assessments, and care plans in a timely and accurate manner.
- Graduate from an approved school of professional nursing and currently licensed to practice as a registered nurse in the state of agency operation.
- One (1) year experience as a RN.
- Licensed to practice as a registered nurse in the state of agency operation.
- Must possess and maintain valid CPR certification while employed in a clinical role.
- Must be a licensed driver with an automobile that is insured in accordance with state and/or organization requirements and is in good working order. Ability to travel to all business locations.
