RN, Inpatient Case Manager, Full Time

We are seeking a skilled RN, Inpatient Case Manager to join our team at Kaiser Permanente. As a key member of our healthcare team, you will work collaboratively with physicians to optimize quality and efficiency of care for hospitalized members.
Responsibilities will include daily review of hospital care, monitoring for inefficiencies and opportunities to improve care, developing safe discharge plans, and recommending alternative levels and sites of care when appropriate.
You will also be responsible for:
- Planning, developing, assessing, and evaluating care provided to members
- Collaborating with physicians, other healthcare team members, and patients/families to develop, implement, and document individualized plans of care
- Reviewing, monitoring, evaluating, and coordinating patients' hospital stays to ensure timely and efficient delivery of essential services
- Communicating with hospitalists and other healthcare team members to ensure seamless transitions of care
- Performing psychosocial assessments on high-risk patients and providing education to assist with discharge and coping with psychological problems related to acute and chronic illness
- Documenting all admissions and discharges in the electronic medical record and making post-discharge follow-up calls to patients
- Attending scheduled rounds with the Physician Director of Resource Stewardship to discuss clinical course and discharge planning for assigned patients
Requirements include:
- Minimum two years of experience in patient care delivery or completion of a Case Management Program
- Associates Degree in Nursing
- Registered Professional Nurse License (Georgia)
- Demonstrated advanced communication and interpersonal skills, ability to collaborate effectively with multidisciplinary healthcare teams, and excellent time management skills
