POSITION: Social Worker

EMPLOYMENT TYPE: Casual

WORK SCHEDULE: Days

DEPARTMENT: Case Management/Social Services


ROLE OVERVIEW:
As a Social Worker, you will deliver expert social services to patients and their families receiving care at Grand Itasca Clinic and Hospital. Your role will involve coordinating healthcare services to ensure optimal outcomes for individuals by utilizing available resources, payor sources, and clinical data. You will address the psychosocial needs of patients and facilitate their medical care to help them achieve their best health status. Acting as a patient advocate, you will assist in identifying and leveraging both internal and external referral sources to meet patient needs. All services provided will adhere to current federal, state, and local regulations. The Social Services department will encourage the effective use of healthcare resources and take on a leadership role within Grand Itasca Clinic and Hospital.

QUALIFICATIONS:
1. A Master's degree in Social Work is preferred.
2. Current licensure in the State of Minnesota through the MN Board of Social Work is required.
3. Experience in acute care settings is preferred.
4. Proven effectiveness in a leadership capacity with the ability to support colleagues and subordinates.
5. Excellent communication skills with patients, families, physicians, and staff are essential.
6. Active participation in the Performance Improvement process is expected.
7. Strong problem-solving skills in challenging situations are necessary.
8. Ability to coordinate admission and discharge planning with the Interdisciplinary Team, patients, and families.
9. Consistent adherence to the NASW Code of Ethics is required.
10. Commitment to following Grand Itasca Clinic and Hospital policies, vision, mission, and values is essential.
11. Reliable and consistent attendance is expected.
