Nurse Practitioner

We are seeking a skilled Registered Nurse to join our team at Baptist Trinity Home Health, a part of LHC Group. As a Registered Nurse, you will have the opportunity to provide high-quality patient care in a home setting, working closely with our interdisciplinary team to deliver comprehensive care.
Responsibilities- Conduct initial and comprehensive nursing evaluations to determine primary focus of care and develop individualized plans of care.
- Coordinate total plan of care with other healthcare professionals, including physicians, therapists, and social workers, to ensure continuity of patient care.
- Provide direct patient care, including administering medications, wound care, and other treatments as prescribed by the physician.
- Collaborate with patients, families, and caregivers to educate them on disease management, medication adherence, and other health-related topics.
- Current RN licensure in the state of practice.
- Current CPR certification.
- Current driver's license, vehicle insurance, and access to a dependable vehicle or public transportation.
