Nurse Practitioner

We're seeking a highly skilled Nurse Practitioner to join our team at ChenMed, a leading healthcare provider dedicated to transforming the lives of seniors and their families. As a key member of our clinical operations team, you will play a vital role in delivering exceptional patient care and contributing to our mission of changing America's healthcare for the better.
Key Responsibilities- Provide direct patient care in our medical centers, including geriatric assessment, medical history, physical exam, diagnosis, and treatment.
- Develop and implement patient management plans, collaborating with other healthcare professionals as needed.
- Participate in clinical rounds and conferences, ensuring seamless communication and coordination of patient care.
- Document patient progress and outcomes, adhering to ChenMed's standards for quality, service, productivity, and teamwork.
- Consult with physicians, managers, and medical directors to ensure compliance with guidelines and best practices.
- ARNP or similar advanced degree in Nursing required.
- Current Nurse Practitioner Certification in the State of practice required.
- Board certification by AANP or ANCC is preferred but may be required for certain States.
- A minimum of 1 year of clinical experience preferred, but willing to consider strong new graduates.
- Basic Life Support (BLS) certification from the American Heart Association (AMA) or American Red Cross required within the first 90 days of employment.
