MDS Coordinator

We are seeking a highly skilled and experienced MDS Coordinator to join our team at Sheffield Manor Nursing and Rehab. As a key member of our care team, you will be responsible for overseeing the Resident Assessment Instrument (RAI) process and conducting assessments and care plan coordination for our guests.
Key Responsibilities- Complete the MDS, CAA's, and care plans within regulated time frames.
- Coordinate scheduling the RAI process with the interdisciplinary team.
- Assess residents through physical assessment, interview, and chart review.
- Discuss resident care needs with caregivers, including physician, nursing, social services, therapy, dietary, and activity staff.
- Review information from hospital, consults, and outside agencies and use such information in the completion of the assessment and care planning.
- Coordinate, identify, and/or initiate significant change MDS.'
- Remain current with American Association of Nursing Assessment Coordinators (AANAC) requirements.
- Registered Nurse (RN)
- AANC certification a plus. RAC-CT
- Knowledge of the Resident Assessment Instrument (RAI) process, including the principles of the Prospective Payment Process (PPS) strongly preferred.
- Experience as an MDS Nurse
